**All fees and charges incurred at TNMA are the full responsibility of the client regardless of insurance coverage. It is the patient's responsibility to know your coverage PRIOR to being seen at TNMA.**

Full payments are due at the time of service unless otherwise arranged by the physician. If you wish to submit a claim through another insurance for services received at TNMA, please request a superbill from the doctor during your visit.

CLINIC POLICIES

**It is TNMA's sole discretion to reserve the right to refuse or limit service to any patient.

**Full payment is due at the time of service for non-insured clients unless otherwise arranged by the physician.

  • Payments Accepted: cash, American Express, Mastercard, Visa, Discover Card

  • Running Late: Please call in advance if you are running late, and note that your treatment time will be shortened accordingly.

  • Denied Payments: denied payments due to insufficient funds will incur a $35 fee billed directly to the client.

  • Late Payments: the full bill will incur 2% interest after 30 days of non-payment and accrue another 2% of the new balance for every 7 days thereafter until the full balance is paid off.

  • Items that are non-refundable/non-returnable:

    • ​prescribed raw herbs

Cancellation/Missed Appointment Policy

Our goal is to provide quality medical care in a timely manner. In order to do so, we have had to implement an appointment/cancellation policy. This policy enables us to better utilize available appointments for our patients in severe pain needing immediate care.

 

Cancellation of an Appointment: 

In order to be respectful of the medical needs of other patients, please be courteous and call the office promptly if you are unable to make an appointment. This time will be reallocated to someone who is in urgent need of treatment. If it is necessary to cancel your scheduled appointment, we require that you call at least 24 hours in advance. Appointments are in high demand, and your early cancellation will give another person the possibility to have access to timely medical care.

How to Cancel Your Appointment: 

To cancel appointments, please call 425-582-2081 or email us at 3team@trinitynma.com. If you do not reach the receptionist you may leave a detailed message on the voice mail. If you would like to reschedule your appointment, please be sure to leave us your phone number and let us know the best time to return your call. 

 

No-Show Policy: 

A "no-show" is someone who misses an appointment without calling 24 hours in advance to cancel. "No-shows" inconvenience those individuals who need access to medical care in a timely manner, as well as the physician. A failure to show up at the time of a scheduled appointment will be recorded in the patient's chart as a "no-show". 

  • 1st missed appointment: No charge to the patient. Courtesy reschedule. 

  • 2nd missed appointment: a fee of $50 will be billed to your account.

  • 3rd missed appointment:  a fee of $50 will be billed to your account and he or she may be asked to leave our practice.

 

Late Cancellations: 

Late cancellations will be considered as a "no-show". We understand sometimes the reasons for late cancellations are beyond the control, so exceptions will only be made in extraordinary circumstances. Cancellations made more than 24 hours in advance of your scheduled appointment time will not be assessed a cancellation fee.

**TNMA has a ZERO TOLERANCE POLICY towards sexual harassment, unwelcome and/or inappropriate physical contact, aggressive behavior, stalking, verbal abuse, suggestive comments or gestures, inappropriate compliments, or any other unprofessional conduct towards its physicians and staff members. Violators will be immediately removed from the building and banned from the clinic indefinitely without possibility of remittance. All incidents will be reported to the authorities and charges may be brought against the offender and severely prosecuted.**
